Compensation: $56.00 - $58.00 per hour

Work Schedule: Monday - Friday; 8:30 AM - 5:00 PM EST

Role Type: Hybrid position requiring travel to member residences.

Mileage Reimbursement: Available

Position Overview: Candidates must possess a valid NY RN license to undertake case management responsibilities. Registered Nurses will receive training on UAS assessments and will conduct visits to members in their homes, assisted living facilities, and nursing homes. Daily tasks will include performing UAS Assessments to facilitate care management, with a minimum of 15 cases assigned weekly.

Key Responsibilities:

  • Conduct in-person evaluations of members' functional status, medical, behavioral, psychosocial, and community resource needs.
  • Provide the Interdisciplinary Care Team (ICT) with assessment data and act as a facilitator to ensure members' needs are addressed.
  • Develop an Individualized Care Plan (ICP) or Person-Centered Service Plan (PCSP) based on members' clinical, behavioral, and social requirements, addressing any barriers to care.
  • Effectively assess members' health status to ensure they receive all necessary medical and supportive services.
  • Adjust the ICP/PCSP as needed based on the member's evolving needs and progress.
  • Manage care transitions through timely and effective communication essential for member care and discharge planning.
  • Clarify medical benefits, policies, and procedures for members, providers, and community-based organizations.
  • Prepare for and engage in clinical case reviews to share best practices.
  • Adhere to documentation policies and procedures.
Qualifications:
  • Valid NYS RN Licensure required.
  • Bilingual in Spanish or Bengali is required.
  • Strong professional knowledge and comprehensive clinical assessment skills in adult populations and chronic disease management.
  • Excellent communication abilities.
  • Proficient in Microsoft Office Suite (Outlook, Word, Excel).
  • Ability to work independently while maintaining flexibility in a fast-paced environment.
  • Capability to analyze data and utilize it to enhance care delivery.
  • Self-motivated with a high level of accountability for care outcomes.
  • Highly organized with the ability to manage multiple priorities effectively.
  • Independent problem-solving skills and the ability to collaborate and build lasting relationships with providers, members, and the multidisciplinary team.
Work Environment Requirements:
  • Must be able to lift at least 30 lbs.
  • Must be able to sit or stand for extended periods (75% of the time).
  • Private, secured internet connection required.
  • Must ensure a safe and secure work environment for handling private and sensitive information.
  • Constantly operates a computer and other office equipment.
  • Primarily works indoors in a secluded, safe space.
  • High-speed internet access of at least 25 MBPS is required; 50 MBPS preferred.
