Job Title: Nurse Case Manager

Grady Health System is seeking a skilled Nurse Case Manager to join our team.

Job Summary

The Nurse Case Manager collaborates with healthcare teams to provide high-quality patient care, ensuring optimal health outcomes and efficient use of resources.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Coordinate care across the continuum, including hospital, home, and community settings
  • Collaborate with healthcare providers to ensure timely and effective care
  • Monitor patient progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare teams
Qualifications
  • Bachelor's Degree in Nursing required
  • Current Georgia Registered Nurse (RN) license
  • Basic Life Support (BLS) certification
  • Minimum 3 years of direct nursing experience and clinical competence
  • Experience in case management preferred
  • Knowledge of age-specific needs of the population served
