About the Opportunity:

Sutter West Bay Medical Group (SWBMG) is actively looking for a dedicated Palliative Medicine Physician to join our esteemed Advanced Organ Therapy (AOT) Clinic. This role offers the chance to work in a vibrant urban environment known for its cultural richness and proximity to various recreational activities.

Location: San Francisco, CA

Key Responsibilities:

Patient Care: Provide comprehensive outpatient evaluations and ongoing care for patients with complex medical conditions such as liver failure, heart failure, or advanced gastrointestinal malignancies. Inpatient coverage is available for those interested in ensuring continuity of care.

Care Coordination: Engage in meaningful discussions about care goals and advance care planning, while assisting with intricate care coordination and supporting medical decision-making for patients and their families.

Symptom Relief: Deliver expert pain and symptom management for patients enrolled in the AOT program.

Collaborative Environment: Become part of a well-established Palliative Medicine department with over 15 years of experience, comprising a team of six physicians across multiple facilities, alongside an inpatient Palliative Care Licensed Clinical Social Worker.

Interdisciplinary Collaboration: Work closely with a diverse AOT team, including community physicians, social workers, chaplains, and administrative staff. Participate in resident education and have opportunities for research involvement.

Technological Integration: Utilize an advanced electronic medical record system (EPIC) that seamlessly connects clinic and hospital operations.

Qualifications:

  • Completion of ACGME Fellowship Training in Hospice and Palliative Medicine, with Board Certification or eligibility.
  • Board Certification in Internal Medicine or Family Medicine.
  • A minimum of two years of post-fellowship clinical experience in Palliative Medicine, particularly in outpatient settings, is highly preferred.
  • Experience with Transplant programs is advantageous.
