Position Summary:
The Hampton Roads Community Health Center, Inc. is in search of a dedicated Registered Dietician to work alongside a multidisciplinary healthcare team. The primary focus of this role is to improve the health and physical wellness of patients through personalized nutritional guidance, particularly in the management of diabetes. This position will report to the Chief Medical Officer.

Key Responsibilities:

  • Assessing patients' health conditions, goals, and dietary limitations
  • Monitoring dietary habits and progress while educating patients on maintaining a nutritious and balanced diet
  • Creating or sourcing educational resources for patient learning
  • Conducting nutritional assessments and counseling to enhance health outcomes
  • Arranging and performing follow-up consultations
  • Facilitating referrals, acquiring lab results, and overseeing medical documentation
  • Formulating and executing personalized nutrition strategies
  • Tracking nutritional status and compliance with treatment plans
  • Providing educational support to medical and nursing personnel
  • Carrying out administrative duties including documentation and data management

Qualifications Required:
A Bachelor's degree in Nutrition or a related discipline, with at least three (3) years of clinical dietetic experience or equivalent. Completion of a state-approved certification program for Registered Dietitians or Certified Diabetic Educators, along with Basic Life Support certification, is essential.
