We are currently looking for an Independent Living Case Manager. In this role, you will engage with clients across various counties, collaborating with our internal treatment team and community organizations to advocate for at-risk children and families. Your responsibilities will include conducting essential needs assessments, providing prevention and intervention services, and monitoring progress.

Key Responsibilities:

Direct Client Services (50%):

  • Assist clients in reaching their personal goals through direct and indirect support.
  • Facilitate connections to necessary resources and services.
  • Provide transportation for clients as needed to support their objectives.
  • Accompany clients to court hearings.
  • Respond to crises based on referral types.
  • Establish and maintain relationships with community agencies.
  • Collaborate with other social service organizations.
  • Refer clients to appropriate community resources.

Documentation and Reporting (50%):

  • Communicate with referring agents and supervisors after each client interaction.
  • Prepare and upload monthly reports to relevant databases.
  • Ensure compliance with case and agency documentation standards.
  • Draft reports for the Department of Child Services and Probation within specified timelines.
Qualifications:
  • A bachelor's degree in social work or a related field is essential.
  • Applicants must be at least 21 years old.
  • This position requires in-person work; remote options are not available.
  • A valid driver's license and insurance are mandatory, with no more than 4 points on the driving record.
  • Familiarity with social service systems within the state is required.
  • Understanding and adherence to confidentiality practices are crucial.
  • Ability to appreciate and respond to cultural differences is necessary.
  • Maintain a network of relevant contacts within the community.
