Care Coordinator for Developmentally Disabled Adults

Job Summary: We are seeking a skilled Care Coordinator to join our team. The successful candidate will play a vital role in providing hands-on support, building strong relationships, and contributing to an environment where both residents and staff can thrive.
Key Responsibilities:
- Provide personalized support to residents, assisting with everyday tasks such as bathing, dressing, and personal hygiene.
- Administer medication in accordance with Title 22 Section 85075.2, ensuring medications are taken as prescribed and stored securely.
- Respond to emergency medical situations promptly, as per Title 22 requirements.
- Encourage residents to participate in community activities, outings, and events.
- Assist residents in developing life skills, including cooking, cleaning, and budgeting.
- Monitor resident behavior, document medication administration, and report concerns to supervisors immediately.
- Maintain accurate records of daily activities, resident progress, and health updates.
- Prepare nutritious meals for residents, tailored to their dietary needs.
- Collaborate with DSPs, supervisors, and healthcare professionals to ensure residents receive the best possible care.
- Participate in training and meetings as directed.
Requirements: To be considered for this role, you must possess a strong desire to help others, excellent communication and teamwork skills, and the ability to work independently and as part of a supportive team. A valid California driver's license and clean driving record are preferred. Certification in Non-Violent Crisis Intervention (NCI), First Aid, and CPR is required. Experience working with developmentally disabled adults and/or human services/behavioral health field is highly desirable. Estimated Salary: $45,000 - $55,000 per year.
