GENERAL OVERVIEW:
The Speech-Language Pathologist operates as an autonomous healthcare practitioner dedicated to delivering tailored patient care that evaluates and addresses disorders related to speech, language, social communication, cognitive communication, voice, fluency, and swallowing in both children and adults upon physician referral. This professional is tasked with assessing referred patients, setting patient and family-centered objectives, and formulating and implementing treatment strategies aimed at enhancing or maximizing independence in the discharge environment.
KEY RESPONSIBILITIES:
- Conduct initial assessments and follow-up evaluations of the patient's condition concerning speech, language, social and cognitive communication, voice, fluency, and/or swallowing in alignment with physician referrals.
- Perform comprehensive evaluations that encompass a review of the patient's medical, surgical, and social history, along with the interpretation of relevant formal and informal assessments across various domains including Motor Speech, Verbal Expression, Written Expression, Voice and Resonance, Auditory Comprehension, Reading Comprehension, Hearing as it relates to speech and language, Swallowing function, Non-Verbal Communication, Orientation, Attention, Problem Solving and Reasoning, Memory, Oral Motor Function, Pragmatics, and Visual Perceptual Skills.
- Develop personalized care plans that synchronize therapist and patient/family-directed goals to achieve realistic outcomes for prevention, intervention, and sustained wellness throughout the patient's life.
- Implement and advance evidence-based therapeutic interventions in accordance with established and revised goals and care plans, adapting as necessary based on the patient's treatment response, while supervising support staff as appropriate.
- Coordinate a comprehensive discharge plan that includes tailored recommendations in collaboration with other healthcare professionals.
- Engage in peer consultations regarding patient care as needed, with formal or informal mentoring expected from more experienced and specialized therapists.
- Maintain educational standards by participating in and providing in-services and staff meetings regularly, along with supporting the orientation of new therapy staff, serving as a Clinical Instructor for students, acting as a clinical fellowship supervisor, and mentoring other clinicians.
- Evaluate the educational needs of patients, address any barriers to understanding or learning, and involve the patient, family, and/or caregiver throughout the therapy process as per the patient's preferences. Additionally, collaborate and communicate with other healthcare team members to ensure optimal therapeutic benefits and quality care.
- Complete necessary medical documentation including initial evaluations, re-evaluations, daily notes, care plans with certification dates, physician correspondence, insurance authorizations, and discharge summaries that substantiate skilled care in accordance with professional, departmental, and payer standards.
- Fulfill administrative responsibilities related to patient care, which may encompass scheduling assistance, monitoring patient attendance, ensuring accurate and timely billing practices, appropriate referral and diagnostic coding, utilization of medically necessary services, compliance with insurance authorizations, participation in peer reviews, performance improvement initiatives, program development, community outreach, and other related activities.
- Contribute to the financial success of the organization by meeting departmental productivity expectations.
- Adhere to work schedule requirements based on site needs and operational hours as mandated for patient care, which may involve flexibility in working at different sites depending on patient volume or other factors.
- Participate in community, philanthropic, and professional presentations.
QUALIFICATIONS:

A. Education / Training:
· Master’s Degree, Certificate of Clinical Competence (ASHA) or Clinical Fellowship Year (CFY), ASHA.
B. Work Experience:
· Experience in evaluating and treating neurological speech and language disorders in both adults and children, including conditions such as aphasia, apraxia, dysarthria, dysphagia, laryngectomy, stuttering, and voice disorders.
· Familiarity with head and neck cancer, tracheostomies, cognitive impairments, traumatic brain injury, dementia, and modified barium swallow studies.
C. Certification, Licensure, Registration:
· Certificate of Clinical Competence from the American Speech-Language-Hearing Association (required) or
· Clinical Fellowship Year (CFY) from the American Speech-Language-Hearing Association.
· Licensure in Speech Pathology in Michigan (required) or a temporary license during the CFY (required).
