Job Title: ChaplainAbout the Role

Bella Home Health and Hospice is seeking a compassionate and skilled Chaplain to join our team. As a Full-Time Chaplain, you will provide spiritual support and guidance to patients and their families, ensuring their emotional and spiritual needs are met with compassion and care.

Key Responsibilities
  • Conduct regular visits to assess patients' spiritual needs and provide personalized support
  • Collaborate with multidisciplinary teams to address holistic care plans and enhance patient experiences
  • Facilitate supportive group sessions and provide one-on-one counseling to foster a sense of community and belonging
  • Document spiritual assessments and care interventions to ensure comprehensive patient support
Requirements
  • Certification as a Chaplain, such as a Degree in Pastoral Ministry
  • Previous experience in hospice or palliative care is highly desirable
  • Strong interpersonal and communication skills to connect with individuals from diverse backgrounds
  • Empathy and ability to handle sensitive and confidential information with discretion
  • Familiarity with relevant software tools for patient documentation and care coordination
