Job Title: Registered Nurse Assessment Coordinator

SpiriTrust Lutheran is a leading provider of senior living services in southcentral Pennsylvania, offering a range of care options including residential living, assisted living, personal care, and nursing and rehabilitation services. We are seeking a highly skilled and compassionate Registered Nurse Assessment Coordinator to join our team at our Village of Sprenkle Drive location in York, PA.

Job Summary:

The Registered Nurse Assessment Coordinator will be responsible for conducting resident assessments, developing and implementing care plans, and coordinating with the interdisciplinary team to ensure high-quality care. This role requires strong clinical skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Conduct resident assessments and develop care plans that meet the individual needs of each resident
  • Coordinate with the interdisciplinary team to ensure seamless care delivery
  • Monitor and evaluate resident progress, making adjustments to care plans as needed
  • Provide education and support to residents and their families on care-related topics
  • Collaborate with other departments to ensure effective communication and coordination of care
Requirements:
  • Current RN license in the Commonwealth of Pennsylvania
  • Minimum of 2 years' experience in a related position
  • Strong clinical skills and knowledge of state and federal regulations
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
