At Talent Masters, we are seeking a highly skilled MDS Coordinator to join our team in a skilled nursing facility in Staten Island. This is a unique opportunity to make a meaningful impact on the lives of our residents and their families.

About the Role:

The MDS Coordinator will be responsible for ensuring accurate assessments and individualized care plans for our residents. This involves overseeing and managing the entire MDS process, performing initial, quarterly, annual, and significant change assessments, and developing, reviewing, and adjusting care plans in collaboration with the interdisciplinary team.

Responsibilities:

  • Ensure all assessments are completed accurately and on schedule to comply with federal and state regulations.
  • Analyze MDS data to optimize care planning and improve quality measures.
  • Develop and implement individualized care plans based on resident assessments.
  • Lead and participate in resident care conferences to foster clear communication between staff, residents, and families.
  • Conduct ongoing training and education for nursing and interdisciplinary staff on MDS requirements, coding, and best practices.
  • Ensure timely submission of MDS data to state and federal agencies.

Qualifications:

  • Active RN or LPN license in New York State.
  • Minimum 2 years of experience as an MDS Coordinator in a skilled nursing facility.
  • Strong understanding of federal and state regulations governing MDS and skilled nursing facility operations.
  • Exceptional organizational skills with the ability to manage multiple assessments and meet tight deadlines.
  • Proficient in electronic health records (EHR) systems and MDS software.
