Position Overview

The Registered Nurse Case Coordinator takes full nursing accountability for implementing the Care Plan for each hospice patient.

This role involves delivering services under the guidance of the interdisciplinary team and is overseen by the Director of Nursing.

Accountability

This position reports directly to the Director of Nursing.

Demonstrates effective time management skills by coordinating activities for maximum productivity and efficiency, adjusting work schedules as needed, seeking prior authorization for overtime, adhering to make-up time policies, and completing assigned projects within specified deadlines.

Participates in Performance Improvement initiatives when requested.

Works well both independently and within the Interdisciplinary Team framework.

Encourages a collaborative approach through cooperation, mutual respect, flexibility, and dependability.

Ensures compliance with all regulatory standards set by State, Federal, and CHAP.

Takes responsibility for ongoing personal development and continuing education.

Completes monthly and annual training as assigned by the Governing Board, Director of Human Resources, and/or Director of Nursing.

Creates opportunities for new referrals and referral relationships while representing the organization in the community.

Supports the philosophy, objectives, and mission of BeLoved Hospice.

Stays current with all mandatory agency requirements.

Adheres to the BeLoved Hospice dress code policy.


Specific Responsibilities:

Implements an individualized Care Plan established by the interdisciplinary team for each patient.

Manages overall care for assigned patients, coordinating all hospice disciplines.

Assists the hospice interdisciplinary team in developing and maintaining the individualized care plan and treatment goals for each assigned patient.

Collects, reports, and analyzes patient care data, implementing care accordingly.

Capable of providing detailed reports on patient conditions to the Chief Medical Officer, Hospice Physician, and interdisciplinary team members as needed.

Immediately communicates any changes to the Chief Medical Officer, Hospice Physician, Director of Nursing, and interdisciplinary team.

Reports unusual conditions, observations, and pertinent information to the Hospice Physician or Director of Nursing.

Understands and applies basic principles of palliative care and pain management within the Scope of Practice.

Complies with all documentation policies and procedures.

Supervises and delegates tasks to Certified Nurse Aides and Licensed Practical Nurses.

Provides intervention and support related to terminal illness and loss.

Offers emotional and spiritual support and makes appropriate referrals to other team members.

Assesses and addresses depression and fear associated with terminal illness.

Facilitates communication among patients, families, and the hospice team.

Identifies caregiver strain that may require respite care relief or support.

Educates and assists patients and families in completing Advance Directives upon request.

Provides personal care as assigned by the Interdisciplinary Team, Chief Medical Officer, Hospice Physician, or Director of Nursing, which may include:

  • Assistance with patient admissions
  • Nursing assessment/reassessment
  • Medication management and administration
  • Comfort measures
  • Insertion/maintenance/removal of foley catheters
  • Insertion/maintenance/removal of various IV catheters for pain control/comfort
  • Application/adjustment of oxygen, IV, NG, G-Tube, and other invasive lines as ordered by the physician
  • Vital signs monitoring
  • Assistance with ambulation/transfers

Adheres to the State of Oregon Nurse Practice Act during all patient care activities.

Follows policy and the Lippincott Manual of Nursing Practice for clinical care.

Facilitates communication among patients, families, and the hospice team.

Performs other duties as assigned.


Qualifications and Experience

To successfully perform this job, an individual must be able to fulfill each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required to perform this job.

  • Must be currently licensed as a Registered Nurse by the State of Oregon Board of Nursing.
  • Preferably have one year of experience as a registered nurse in hospice, home health, or geriatric care.
  • Must be able to speak, read, and write in the English language.
  • Must have reliable transportation and maintain auto insurance.
  • Must hold Basic Life Support (BLS) Certification.
  • Successful completion of a background check pursuant to ORS 181A.195.
  • Must be able to manage frequent interruptions, problem-solve, and demonstrate exceptional follow-through skills.
  • Must be able to cope with emotionally charged situations involving patients, families, caregivers, and team members.

Physical/Safety Requirements:

Participates in emergency procedures, including disaster drills.

Operates equipment safely and shares responsibility for communicating safety violations or unsafe practices to management.


Equipment Used:

General office equipment (telephone, copy machine, computer, fax machine) and motor vehicles.

Category 1 for potential exposure to blood/bodily fluids.

Follows the designated plan of action in the event of a fire or other emergency/disaster.

Must possess sight/hearing/speech senses or use prosthetics that will enable these senses to function adequately to meet the requirements of this position.

Uses appropriate body mechanics as needed for personal use.

Utilizes basic office equipment effectively.
