A-Line Staffing Solutions is seeking a dedicated Registered Nurse Care Coordinator. This role offers the opportunity to work with a leading organization in the healthcare sector, providing significant potential for professional advancement. The position is structured as full-time, requiring 40+ hours per week.
Compensation and Benefits
- The hourly wage for this role ranges from $42 to $46.
- Full-time employees are eligible for benefits after a 90-day introductory period.
- A 401(k) plan with company matching is available for full-time staff after one year of service.
Position Highlights
- The standard working hours for this position are Monday through Friday, from 8 AM to 5 PM.
Key Responsibilities
- Conduct care management tasks to evaluate, plan, and coordinate comprehensive medical and ancillary services for selected clients, ensuring quality and cost-effective care.
- Perform client assessments and maintain all necessary documentation, including contractual outreach and follow-ups on outstanding client needs.
- Engage with clients through phone communications for contractual outreach and conduct in-home visits as required.
- Assist clients with various needs, including Durable Medical Equipment (DME) and waiver services.
- Evaluate community members through Waiver assessments and facilitate authorization placements.
Qualifications
- Must possess a valid TX Registered Nurse license.
- Experience in Case Management is essential.
- Strong assessment skills are required.
- Excellent organizational abilities are necessary.
- Consistent attendance is required during the initial 90 days of employment.
Preferred Qualifications
- Experience in Case Management.
- Familiarity with Health Plans.
- Background in Home Health services.
