Job Title: LPN Clinical Support Specialist Home Health

Overview: AccentCare is seeking a skilled LPN Clinical Support Specialist to join our team in the Magee, MS area. As a Clinical Support Specialist, you will play a vital role in providing world-class patient care and creating incredible memories for our patients and their families.

Responsibilities:
  • Provide clinical support to patients in their homes, ensuring their safety and well-being.
  • Collaborate with healthcare teams to develop and implement effective care plans.
  • Conduct thorough assessments and evaluations to identify patient needs.
  • Develop and maintain strong relationships with patients, families, and healthcare providers.

Requirements:
  • RN or LVN/LPN with current licensure to practice nursing in the practicing state.
  • Possess and maintain CPR certification.
  • Previous experience in home care setting preferred.
  • Prior use of Electronic Medical Records (EMR) preferred.
