Physical Therapist Home Health Specialist

AccentCare, Inc. is seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in providing high-quality patient care and improving the lives of our patients.
Key Responsibilities:
- Provide physical therapy services to patients in their homes, focusing on improving mobility, strength, and function.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and goals.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
- Master's degree in physical therapy from a CAPTE-accredited program.
- Current licensure to practice as a physical therapist in the state of operation.
- Valid CPR certification.
- Ability to travel to all business locations.
