Home Health Registered Nurse

We are seeking a highly skilled and compassionate Home Health Registered Nurse to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.
Responsibilities- Provide comprehensive skilled nursing visits to home health patients, including admission, case management, and follow-up care.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Confer with physicians and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Administer ongoing care and case management, providing necessary follow-up as directed by the Clinical Manager.
- Monitor patient progress, revise care plans as needed, and ensure timely documentation of patient care and outcomes.
- Collaborate with interdisciplinary teams, including physical therapists, occupational therapists, speech-language pathologists, and home health aides, to provide comprehensive care.
- Coordinate and oversee the work of home health aides, certified home health aides, and personal care workers, ensuring high-quality care and adherence to care plans.
- Prepare and complete necessary clinical documentation, including visit reports, patient records, and care plan updates.
- Diploma, Associate, or Bachelor's Degree in Nursing.
- Minimum of one year nursing experience, with strong med-surg, ICU, ER, and acute care experience preferred.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills, with ability to work effectively in a fast-paced environment.
- Valid driver's license, auto insurance, and reliable transportation.
