Family Housing Corp. In this position, you will collaborate with a team of dedicated professionals to conduct outreach assessments, provide counseling, and facilitate referral services for those in need.

Key Responsibilities:

  • Conduct mobile outreach to engage with individuals experiencing homelessness
  • Offer emotional encouragement and support to motivate individuals to pursue assistance
  • Establish connections with treatment providers and oversee referral processes
  • Deliver housing-oriented case management utilizing a Harm Reduction framework
  • Design personalized treatment strategies for clients with substance use challenges
  • Ensure the completion of referrals and advocate for clients as necessary
  • Work collaboratively with outreach team members and maintain precise documentation
  • Participate in program-related activities and team meetings
  • Utilize vehicles for outreach efforts within the community
  • Perform additional tasks as directed by the supervisor

Qualifications:

  • Required accreditation: CADAC or CAADE
  • Experience in counseling or working with homeless populations
  • Ability to successfully complete TB screenings and physical assessments
  • Possession of a valid California Driver's License and auto insurance
  • Willingness to work on occasion during weekends
