Sugar Maple Health is seeking a skilled Physical Therapist to join our Home Care team in Newark, NJ. As a Physical Therapist in this role, you will work with a geriatric population in their independent homes or within senior living/assisted living facilities.
Key Responsibilities- Provide high-quality physical therapy services to patients in their homes or in senior living facilities
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure comprehensive care
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions
- Competitive salary range starting at $80,000 per year based on experience
- Flexible scheduling to accommodate your needs, with a minimum of 12 hours of direct patient care and 3 weekdays of availability per week
- A comprehensive benefits package, including health, dental, and vision insurance, as well as retirement savings options
- Ultimate schedule flexibility, allowing you to work when you want and see as many patients as you want
- Current licensure as a Physical Therapist in the state of New Jersey
- Experience working with geriatric populations, preferably in a home care or senior living setting
