Job Summary:
As a Social Services Coordinator with SSM Health, you will work with an interdisciplinary team to develop and provide a program of services and referrals for individuals experiencing bereavement. This role requires a Master's degree in Social Work or a related field and one year of experience in a similar position.

Responsibilities:
• Coordinate initial assessments and develop plans of care
• Organize and coordinate grief-related support groups
• Train volunteers to work in program development and directly with the bereaved
• Develop grief education programs and materials for clients and community distribution
• Serve as a community liaison for bereavement outreach and education
• Ensure visits and interventions are documented according to regulatory standards

Requirements:
• Master's degree in Social Work or related field
• One year of experience in a similar position

Physical Requirements:
• Frequent reaching, sitting, standing, and walking
• Frequent use of vision and depth perception for distances near and far
• Frequent use of hearing and speech for oral communication
• Occasional bending, stooping, climbing, twisting, and gripping
• Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs
• Occasional use of vision to identify and distinguish colors
• Occasional driving
• Rare repetitive foot/leg and hand/arm movements and squatting
