Job Summary

Responsibilities
- Conduct thorough psychosocial assessments of patients and develop personalized care plans that meet their unique needs.
- Provide emotional support and counseling to patients and their families, addressing issues such as stress, anxiety, and grief.
- Work collaboratively with other healthcare professionals to ensure seamless delivery of patient care.
- Document all patient interactions and care plans in a timely and accurate manner.
- Assist patients and their families in accessing community resources and benefits, such as Medicaid and Supplemental Security Income.
Requirements
- A Bachelor's degree in Social Work (BSW) or a Master's degree in Social Work (MSW)
- Current State licensure in Social Work (LSW or LCSW) preferred
- A minimum of one year's experience in a healthcare setting, preferably in hospice care
- A valid driver's license and proof of insurance
