Responsibilities



  • Conduct thorough psychosocial assessments of patients and develop personalized care plans that meet their unique needs.
  • Provide emotional support and counseling to patients and their families, addressing issues such as stress, anxiety, and grief.
  • Work collaboratively with other healthcare professionals to ensure seamless delivery of patient care.
  • Document all patient interactions and care plans in a timely and accurate manner.
  • Assist patients and their families in accessing community resources and benefits, such as Medicaid and Supplemental Security Income.

Requirements



  • A Bachelor's degree in Social Work (BSW) or a Master's degree in Social Work (MSW)
  • Current State licensure in Social Work (LSW or LCSW) preferred
  • A minimum of one year's experience in a healthcare setting, preferably in hospice care
  • A valid driver's license and proof of insurance
