Senior Hospice Nurse Practitioner

We are seeking a compassionate and skilled Registered Nurse to join our team at Lightways Hospice and Serious Illness Care. As a key member of our home healthcare team, you will provide high-quality patient care in the comfort of their own homes.
The ideal candidate will have a minimum of one year of experience in either home health or nursing home/long-term care settings. Previous hospice experience is highly desirable.
This full-time position covers Lake and Northern Cook County areas and requires a strong commitment to delivering exceptional patient-centered care. Our RN Case Managers work Monday through Friday and must be available for a minimum of 20 hours of on-call per month, which may increase based on staffing needs.
