Hospice Support Specialist

We are looking for an experienced Hospice Support Specialist to join our team at Lightways Hospice and Serious Illness Care. This part-time position will involve traveling to patients' homes and other locations to provide counseling and support services. The estimated salary for this role is $40,000-$50,000 per year, based on industry standards and location.
In this role, you will work closely with patients, families, and caregivers to provide emotional support and help them navigate the healthcare system. You will also be responsible for developing and maintaining relationships with referral sources and providing information to patients and families as needed.
To be successful in this role, you will need a Social Worker MSW, LSW preferred. BSW accepted with a required minimum of one year experience in either a hospital setting, home health or nursing home/long term care. Hospice experience is highly desirable.
