The Department of Pharmaceutical Services at UCSF provides a comprehensive range of services for patients cared for by the Medical Center and its providers and payors. These services include drug procurement, storage, and security; safe, accurate, and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs.
The Staff Pharmacist II - Acute Care Float Pool position provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed.
Responsibilities- Obtain medication histories and review and assess medication profiles
- Provide drug information to patients and health care providers
- Collaborate with clinicians on drugs of choice and therapeutic plans
- Ensure access to medications and educate patients on proper use
- Ensure patients understand cautions, adverse reactions, and interactions related to their therapy
- Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy
- Completion of an ASHP-accredited PGY1 Pharmacy Residency, or at least 3 years of inpatient/operational pharmacy experience
- Hold an advanced practice board certification (e.g., Board of Pharmacy Specialty certificates, AAHIVP, etc.)
- Must possess a current license in good standing to practice pharmacy in the State of California
- Current certification for Basic Life Support (BLS) from the American Heart Association
