Key Responsibilities :

The Palliative Care Nurse Coordinator (RN) plays a pivotal role within our clinical team, exemplifying professional nursing care in alignment with established care plans. The RN is dedicated to delivering high-quality care during patient interactions and ensuring meticulous documentation. Compliance with agency policies and all relevant professional standards is paramount.

  • Conducts thorough assessments of patient conditions, encompassing physical, psychosocial, and environmental factors
  • Ensures adherence to federal and state regulations governing hospice services through consistent policy application
  • Develops individualized care plans based on assessment data, with regular updates at least bi-weekly
  • Implements skilled interventions aimed at achieving realistic patient outcomes within designated timeframes
  • Performs nursing interventions within the legal scope of practice
  • Maintains comprehensive and accurate clinical documentation throughout the patient care continuum
  • Demonstrates proficiency in technical nursing skills in accordance with legal standards
  • Provides tailored education to patients and families, adjusting as necessary
  • Reviews clinical records as required
  • Undertakes on-call duties as assigned
  • Conducts patient visits efficiently, ensuring timely documentation submission per branch protocols
  • Assesses patient eligibility for admission and assists in staff assignments
  • Collaborates with physicians to refine care plans based on patient needs
  • Prepares clinical progress notes and continuously reviews documentation for compliance
  • Ensures the availability of necessary medical supplies and equipment for assigned patients
  • Adheres to patient care policies and protocols
  • Provides supervisory and educational visits for professional and paraprofessional staff, offering feedback to management as needed
  • Facilitates orientation and training for new staff to ensure quality care delivery
  • Reviews HHA documentation for completeness and appropriateness
  • Ensures compliance with state licensure requirements and agency policies
  • Proposes policy enhancements to improve patient service quality and operational efficiency
  • Completes additional assignments as directed to meet company objectives
