Palliative Care Nurse Practitioner

We are seeking a skilled Palliative Care Nurse Practitioner to join our team at Freda H. Gordon Palliative Care, a part of LHC Group. As a Palliative Care Nurse Practitioner, you will provide direct patient and family care through palliative care consultation to ensure specialty services to patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.
Responsibilities:- Provide direct patient and family care through palliative care consultation
- Ensure specialty services to patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care
- Demonstrate effective and ethical use of approved evidence-based palliative medicine practices in the delivery of all patient care
- Formulate an effective plan of care incorporating palliative care body of knowledge
- Conduct thorough specialty physical, emotional, psychosocial, and spiritual clinical patient assessment
- Complete a comprehensive assessment utilizing the palliative care domains of assessment, interpret findings, conduct medication reconciliation, and formulate problem-related diagnoses
- RN/APRN licensure in state of practice
- Nurse Practitioner/CNS with prescriptive authority
- 5 years of clinical nursing experience, with at least 1 year as APRN with Hospice, Palliative Care or Oncology preferred
- Advanced practice palliative care certification (ACHPN) is preferred
