Palliative Care Nurse Practitioner

**Job Summary:**
We are hiring a Palliative Care Nurse Practitioner to provide direct patient and family care through palliative care consultation. The ideal candidate will have a strong background in palliative care and a passion for delivering high-quality patient care.
**Responsibilities:**
* Provide direct patient and family care through palliative care consultation
* Develop and implement individualized care plans to meet the unique needs of each patient
* Collaborate with the interdisciplinary team to ensure comprehensive care
* Conduct thorough assessments and develop effective treatment plans
* Educate patients and families on palliative care options and provide support
* Participate in quality improvement initiatives to enhance patient care
**Requirements:**
* RN/APRN licensure in the state of practice
* Nurse Practitioner/CNS with prescriptive authority
* 5 years of clinical nursing experience, with at least 1 year in palliative care or a related field
* Advanced practice palliative care certification (ACHPN) is preferred
