Overview

Position Summary: Under the guidance of the pharmacy manager and/or pharmacy supervisor, this role encompasses all necessary functions to ensure the smooth daily operations of the pharmacy, delivering high-quality products and services to patients receiving infusion therapy.

Responsibilities Overview: Accountable for the execution and compliance with KPH Healthcare Services policies to guarantee the highest standard of healthcare services for our patients.

Key Responsibilities

Core Duties:

  • Ensures adherence to federal, state, and local laws, regulations, policies, and standards set by accrediting bodies.
  • Reviews and assesses all physician medication orders to ensure therapeutic appropriateness, cost-effectiveness, accuracy, and completeness.
  • Maintains all legally mandated records with precision.
  • Directly oversees pharmacy support staff and/or interns in the correct preparation, packaging, and labeling of pharmaceuticals.
  • Demonstrates proficiency in CPR software usage.
  • Aids in the training and onboarding of new staff members.
  • Provides technical and clinical information, communicating monitoring results to patients, nurses, and physicians as necessary regarding drug contents, usage, therapeutic effects, and potential adverse reactions or interactions.
  • Initiates, reviews, and updates all required documentation in accordance with KPH policies and procedures.
  • Follows established protocols for DME and supply procurement, maintaining suitable inventory levels and preferred product selections of drugs, solutions, and supplies.
  • Accurately prepares IV admixtures, utilizing appropriate techniques and performing necessary checks per policies and procedures.
  • Complies with stability and compatibility guidelines along with quality control measures.
  • Ensures the security of all legend items, including controlled substances.
  • Executes precise computer order-entry, labeling, and dispensing tasks.
  • Collaborates with nurses and physicians in all clinical activities as required, including the initial review of medical and drug histories, pharmacokinetic dosing, dosage monitoring for at-risk patients, reviewing lab results, attending patient care meetings, and developing/updating patient-specific plans of care.
  • Engages in the referral acceptance/denial process and selection of products, devices, and supplies.
  • Participates in on-call rotations as needed.
  • Involved in Performance Improvement initiatives, educational programs, and departmental meetings.
  • Advocates for the Primary Case Management Model (PCMM) in daily interactions by delegating non-cognitive tasks to support personnel, ordering only clinically necessary and cost-effective labs, and facilitating positive patient outcomes with patients, nursing agencies, and other healthcare providers.
  • Undertakes additional duties as assigned.
  • Acts as Process Owner as designated.
  • Serves as compliance officer.
  • Responsible for completing all mandatory and regulatory training programs.

Qualifications

Educational Background:

  • Bachelor's Degree (BS) in Pharmacy or a Pharm.D. from an accredited institution.

Experience:

  • Preferably one year of experience in an IV alternate site environment or equivalent experience.

Licensure Requirements:

  • Current license for the state of practice.
  • Current CPR certification if involved in direct patient care.
  • Valid driver's license and appropriate automobile insurance as per KPH policies.

Employment Conditions:

  • Criminal background check and/or drug screening.
  • Initial and ongoing exclusion and sanction/disciplinary monitoring.

Required Training:

  • HIPAA Privacy Course.
  • HIPAA Security Course.

Job Skills Required:

  • Ability to read, analyze, and interpret various documents, including business periodicals, professional journals, technical procedures, or governmental regulations. Capable of writing reports, business correspondence, and procedural manuals. Proficient in presenting information and responding to inquiries from groups of managers, patients, nurses, and physicians.
  • Competent in basic arithmetic operations and capable of solving word problems and algebraic equations. Must be able to perform accurate pharmaceutical calculations.
  • Skilled in problem-solving and handling a variety of concrete variables in situations with limited standardization. Ability to define problems, gather data, establish facts, and draw valid conclusions. Proficient in interpreting a variety of instructions in written, oral, diagram, or schedule form. Computer skills in word processing, database management, and spreadsheet applications are essential.

Compensation:

$132,042.06 per year.

The final offer will be determined after careful consideration of multiple factors such as relevant skills, years of experience, and education.
