Position Overview

Job Summary: Under the guidance of the pharmacy manager and/or pharmacy supervisor, this role is essential for ensuring the effective daily functioning of the pharmacy, delivering high-quality products and services to patients receiving infusion therapy.

Scope of Responsibilities: Accountable for the execution and compliance with KPH Healthcare Services policies to guarantee the highest standard of healthcare service for our patients.

Key Responsibilities

Core Duties:

  • Ensures adherence to federal, state, and local laws, regulations, policies, and standards set by accrediting bodies.
  • Analyzes and assesses all physician medication orders to confirm therapeutic suitability, cost efficiency, accuracy, and completeness.
  • Maintains all legally mandated records with precision.
  • Directly oversees pharmacy support staff and/or interns in the correct preparation, packaging, and labeling of pharmaceuticals.
  • Demonstrates proficiency in CPR software.
  • Aids in the training and onboarding of new personnel.
  • Provides clinical and technical information, communicating monitoring results to patients, nurses, and physicians regarding drug contents, usage, therapeutic effects, and potential adverse reactions or interactions.
  • Initiates, reviews, and updates all required documentation as per KPH policies and procedures.
  • Follows established protocols for DME and supply procurement, maintaining optimal inventory levels and preferred product selections.
  • Accurately prepares IV admixtures, employing appropriate techniques and performing necessary checks.
  • Complies with stability and compatibility guidelines and quality control measures.
  • Ensures the security of all legend items, including controlled substances.
  • Executes precise computer order-entry, labeling, and dispensing.
  • Collaborates with nurses and physicians in clinical activities, including initial care reviews, pharmacokinetic dosing, dosage monitoring for at-risk patients, and developing/updating patient-specific care plans.
  • Engages in the referral acceptance/denial process and selection of products, devices, and supplies.
  • Participates in on-call rotations as needed.
  • Contributes to Performance Improvement initiatives, educational programs, and departmental meetings.
  • Promotes the Primary Case Management Model (PCMM) through effective delegation of non-cognitive tasks to support personnel, ensuring clinically necessary and cost-effective lab orders, and facilitating positive patient outcomes.
  • Undertakes additional duties as assigned.
  • Acts as Process Owner as designated.
  • Serves as compliance officer.
  • Completes all mandatory and regulatory training programs.

Qualifications

Educational Background:

  • Bachelor's Degree (BS) in Pharmacy or a Pharm.D. from an accredited institution.

Experience:

  • Preferred one year of experience in an IV alternate site setting or equivalent experience.

Licensure Requirements:

  • Current state license for practice.
  • Current CPR certification if involved in direct patient care.
  • Valid driver's license and appropriate automobile insurance as per KPH policy.

Employment Conditions:

  • Criminal background check and/or drug screening.
  • Initial and ongoing exclusion and sanction/disciplinary monitoring.

Required Training:

  • HIPAA Privacy Course.
  • HIPAA Security Course.

Job Skills Required:

  • Ability to read, analyze, and interpret various professional documents and regulations. Capable of writing reports, business correspondence, and procedural manuals. Effectively presents information and responds to inquiries from diverse groups.
  • Proficient in mathematical calculations, including addition, subtraction, multiplication, and division in various units of measure. Ability to compute rates, ratios, and percentages, and interpret graphs.
  • Skilled in problem-solving and dealing with diverse variables in non-standardized situations. Able to collect data, establish facts, and draw valid conclusions.
  • Competent in computer applications (word processing, databases, spreadsheets, etc.).

Compensation

$132,042.06 per year.

The final offer will be determined after careful consideration of various factors such as relevant skills, experience, and education.
