GENERAL OVERVIEW

Engages in moderately complex to advanced case management responsibilities aimed at fostering and sustaining long-term relationships with assigned youth, their families, and service providers addressing medical, social, educational, and related needs. Responsibilities include conducting case file evaluations, developing case plans, involving families, delivering group services, establishing objectives, and assessing outcomes. This role also entails managing discharge and release processes to ensure seamless service coordination throughout the youth's commitment period.

Career Advancement: This position is part of the established TJJD Case Manager career trajectory. New hires will be placed at a level that aligns with their educational background and experience, with opportunities for promotion to Case Manager IV based on meeting educational and performance criteria as outlined by agency policy.

Pre-Service Training Academy Requirement: Newly appointed case managers or those returning after a significant separation will be required to complete the pre-service training academy. Individuals who have previously worked for TJJD and return within a specified timeframe will not need to attend this training and will be classified as full-time employees upon their start date.

Levels of Work Performed:

Case Manager II: Engages in moderately complex tasks under general supervision, exercising moderate discretion and independent judgment.

Case Manager III: Handles highly complex tasks under limited supervision, utilizing considerable discretion and independent judgment.

Case Manager IV: Undertakes advanced and/or supervisory tasks under minimal supervision, with extensive discretion and independent judgment.

KEY RESPONSIBILITIES

1. Conducts initial assessments and file reviews, requests necessary documentation, performs ongoing evaluations for assigned cases. (20%)

2. Develops case plans for assigned youth, applying suitable strategies to promote progress and facilitate specialized referrals. (15%)

3. Monitors caseload to ensure effective transition planning. (10%)

4. Facilitates required group sessions for assigned youth and plans for scheduled and unscheduled absences to ensure continuity of services. (15%)

5. Participates in professional development, including approved treatment programs and case management training. (5%)

6. Provides individual counseling to youth and family contacts. (10%)

7. Completes necessary documentation of services and maintains accurate case records. (10%)

8. Reviews and applies relevant information from youth records concerning health, safety, and facility security. (5%)

9. Adheres to policies and procedures, preparing and maintaining required records and reports. (5%)

10. Reports any perceived disparities related to race, ethnicity, or gender.

11. Performs additional duties as assigned, ensuring compliance with agency safety policies and procedures.

PHYSICAL DEMANDS AND WORKING CONDITIONS

Ability to communicate effectively, perform physical tasks, and engage in activities such as lifting, carrying, and restraining youth when necessary. Must be capable of working in a dynamic environment and responding to emergencies.

KNOWLEDGE, SKILLS, & ABILITIES

Knowledge of child development, case management principles, and effective communication strategies. Skills in developing and implementing treatment plans, maintaining records, and establishing effective working relationships.
