Registered Nurse Diabetes Coordinator

We are seeking a highly skilled Registered Nurse Diabetes Coordinator to join our team at Del Sol Medical Center. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and education to individuals with diabetes.
Job Summary:The Registered Nurse Diabetes Coordinator will be responsible for:
- Providing comprehensive diabetes education and support to patients and their families
- Collaborating with healthcare providers to develop and implement individualized care plans
- Conducting screenings and assessments to identify patients at risk for diabetes-related complications
- Developing and implementing educational programs to promote healthy lifestyle choices and disease management
- Providing ongoing support and guidance to patients and their families to ensure optimal diabetes management
To be successful in this role, you will need:
- A Bachelor of Science in Nursing (BSN) degree and current RN licensure
- Minimum 5 years of experience in a healthcare setting, with a focus on diabetes care and education
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
- Knowledge of diabetes management principles and practices, including medication management and insulin therapy
