Registered Nurse Case Manager

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Del Sol Medical Center. As a key member of our healthcare team, you will play a vital role in promoting patient-centered care by coordinating the plan of care for patients, managing length of stay, and ensuring appropriate resource management.
Key Responsibilities- Provide comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as an assessment of available resources.
- Develop and implement individualized care plans that address patients' strengths, abilities, needs, and preferences.
- Collaborate with the multidisciplinary healthcare team to facilitate a successful care transition and ensure timely referrals to social services when necessary.
- Document professional recommendations, discharge plans, care coordination interventions, and case management activities to effectively communicate with the healthcare team.
- Act as a liaison between patients, families, hospital staff, and outside agencies to ensure seamless communication and coordination of care.
- Identify and provide for the needs of under-resourced patient populations, including patient education activities, patient assistance programs, and community-based resources.
- Participate in performance improvement activities, including identifying, documenting, and intervening when avoidable days occur.
- Associate Degree in Nursing or Nursing Diploma Required
- Bachelor's Degree in Nursing Preferred
- 2+ years experience in case management or 3+ years experience in clinical nursing Required
- InterQual experience Preferred
- Certification in Case Management Preferred
- Ability to establish and maintain collaborative and effective working relationships
- Ability to communicate effectively in oral, written, and electronic formats
- Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills
