The LPN is accountable for triaging patient care both in the clinic and via telephone. Responsibilities include gathering the patient's chief complaint, taking a concise medical history, measuring vital signs and growth parameters, performing basic laboratory tasks, assisting the provider as necessary, and managing patient flow within the clinic. This role operates under the direct supervision of a physician or provider.
Specific Duties:
- Administers nursing care activities in accordance with established patient care protocols and standards.
- Admits patients for care, ensuring accurate history and chief complaint documentation.
- Provides essential data to aid in the formulation of nursing diagnoses central to the care plan.
- Contributes to the development of strategies to meet patient and family needs, creating patient care plans using standardized nursing protocols and prioritizing care effectively.
- Implements planned care approaches and performs common therapeutic nursing techniques.
- Collaborates with registered nurses to adjust care plans and reports care outcomes.
- Engages in patient and family education, documenting teaching efforts with appropriate resources.
- Assists in effective discharge and referral planning upon admission, utilizing relevant hospital and community resources.
- Delegates patient care tasks to appropriate personnel while maintaining accountability.
- Ensures all patient care activities optimize safety and minimize the risk of healthcare errors; documents according to established standards and communicates effectively.
- Continually develops and maintains professional and clinical skills, completing all required competencies and educational activities within designated time frames. Participates in staff meetings, patient care discussions, and other professional development activities while staying informed of policies and performance standards.
- Performs additional duties as assigned.
- High school diploma or equivalent.
- Graduation from an accredited Licensed Practical Nurse program.
- Licensed Practical Nurse license in the State of Washington, as required for the position.
- Current Basic Life Support (BLS) certification, or to be obtained within 180 days of hire.
- Minimum of one year of experience in an acute care setting.
