We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Fir Lane Health & Rehabilitation Center, a Caldera Care facility. As an LPN, you will play a vital role in providing top-notch care to our residents, delivering the highest standards of excellence.

Job Responsibilities
- Provide nursing care to patients as assigned, administering and recording medications and treatments as needed.
- Reinforce supports and provide health care instructions to patients and their families, promoting a smooth transition to home care.
- Participate with other team members in planning, implementing, and evaluating nursing care and other patient activities, ensuring a comprehensive approach to care.
- Assist in the admission, transfer, and discharge of patients, ensuring a seamless experience for residents and families.
- Observe and report patients' vital signs, symptoms, and general condition to the appropriate individual(s), ensuring timely interventions.
- Record on patient charts observation of patient's responses to nursing care rendered, maintaining accurate and up-to-date records.
- Assist with unit management and/or assume charge of unit as assigned, demonstrating leadership skills and a commitment to excellence.
Qualifications and Skills
- Current and Valid LPN license, with a strong understanding of patient care policies and regulations.
- Excellent interpersonal skills, with the ability to communicate effectively with patients, families, and healthcare teams.
- Fast and adaptive problem-solving abilities, with a focus on delivering high-quality care in a dynamic environment.
- Physical strength and stamina, with the ability to perform tasks that require standing, walking, and lifting.
- First aid and CPR certifications, with a commitment to maintaining current certifications.
