About the Role

Rising Ground is seeking a skilled Social Worker to join our team in Brooklyn/Bronx. As a Social Worker, you will be responsible for providing casework services to residents of the Runaway Homeless Youth Shelter TIL Program.

Key Responsibilities
  • Provide casework services to residents, including referrals for entitlements, housing, and services.
  • Accompany residents on appointments and foster relationships with other service providers.
  • Develop Individualized Service Plans for youth in collaboration with the treatment team.
  • Develop discharge plans and support networks for residents.
  • Provide individual and group counseling in the facility.
  • Coordinate and monitor multi-disciplinary service provision.
  • Ensure the safety of all youth on caseload.
  • Maintain daily communication with childcare staff and Program Administrator.
  • Assist residents with Independent Living preparation, employment, and educational services.
  • Maintain up-to-date case record documentation and reports.
Requirements
  • MSW/Master's Degree
  • Excellent writing skills
  • Computer, Word/Excel/Microsoft
  • Valid Driver's License
