Primary Care Physician Leader

ChenMed is a physician-led and mission-driven organization that is changing the face of healthcare for seniors. As a Primary Care Physician Leader, you will be part of a team that is dedicated to providing exceptional care to our patients and making a meaningful impact on their lives.
Key Responsibilities:- Provide high-quality patient care and lead a team of healthcare professionals to achieve exceptional outcomes
- Develop and implement clinical strategies to improve patient health and well-being
- Collaborate with other healthcare providers to ensure seamless care coordination
- Lead Super Huddle and Transforming Care Meeting weekly, and support Center Manager/Center General Manager as needed
- Monitor daily center census and participate in joint center accountability for outcomes
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
- Current DEA number for schedule II-V controlled substances
- BLS certification from the American Heart Association or American Red Cross
