Primary Care Physician Leader

ChenMed is a physician-led and mission-driven organization that is changing the face of healthcare for seniors. As a Primary Care Physician Leader, you will be part of a team that is dedicated to providing exceptional care to our patients.
Key Responsibilities- Provide high-quality patient care, including assessment, diagnosis, treatment, and management of acute and chronic health needs.
- Lead a team of care professionals, including care promoters, care facilitators, and care coordinators, to ensure seamless patient care.
- Develop and implement care plans that are tailored to the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Contribute to the development and implementation of clinical policies and procedures.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current State Medical License or ability to obtain one.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC).
