Medical Social Worker

We are seeking a skilled Medical Social Worker to join our team at CenterWell Home Health. As a Medical Social Worker, you will play a vital role in providing comprehensive care to our patients in the comfort of their own homes.
Key Responsibilities- Evaluate and develop personalized plans of care to address patients' emotional and social needs.
- Assess patients' social and emotional states, identifying areas where support is needed.
- Provide direction and supervision to Social Worker Assistants as required.
- Collaborate with healthcare teams to ensure seamless care coordination.
- Participate in discharge planning and identify community resources to support patients.
- Document patient progress and maintain accurate records.
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Social Worker licensure in the state of practice (if required by state law or regulation).
- Valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
- Excellent oral and written communication and interpersonal skills.
- Ability to read, write, and speak fluent English.
