Job Summary
The Licensed Practical Nurse (LPN) will play a vital role in delivering high-quality patient care, education, and support to our geriatric patients. As a member of our interdisciplinary team, you will work closely with our physicians to develop and implement personalized care plans that address the physical, psychological, and social needs of our patients.

Key Responsibilities
Assist the provider in collecting patient data and implementing age-appropriate interventions based on individual patient needs
Provide ongoing education and reinforcement of care plans developed by the provider
Assist in evaluating the effectiveness of care given and progressing patients toward desired outcomes
Demonstrate teamwork in the delivery of patient care
Assist the provider with triaging patient portal messages and providing feedback to patients

Requirements
Graduated from an accredited LPN program
Licensed to practice as a Practical Nurse in the State of Maryland
Minimum 5 years of experience in geriatric care
CPR and first aid certification required
Experience with EMR and phlebotomy
Professional demeanor and communication skills
