Job Description for Care Manager Position

Job Summary: We are seeking a highly motivated and detail-oriented Care Manager to join our team. The successful candidate will be responsible for conducting assessments, developing care plans, and coordinating with healthcare providers to ensure the best possible outcomes for our residents.

Key Responsibilities
  1. Assessment and Care Planning: Conduct thorough assessments of residents' needs, develop personalized care plans, and update them as necessary.
  2. Care Coordination: Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination and address any concerns or issues that may arise.
  3. Communication and Education: Provide education and support to residents and their families on various aspects of healthcare and wellness, and communicate effectively with all parties involved.
  4. Record Keeping and Compliance: Maintain accurate records of residents' progress and ensure compliance with regulatory requirements and PRC's policies and procedures.
Requirements and Qualifications
  • Education: Bachelor's degree in psychology, sociology, human services, or a related field.
  • Experience: 1-2 years' experience in human services, preferably in geriatric care.
  • Skill Set: Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders; basic computer skills, including proficiency in Windows, Outlook, and Microsoft Office.

Salary Range: $60,000 - $80,000 per annum, depending on qualifications and experience.
