Job Description:

The Community Renewal Team is seeking a skilled and dedicated Healthcare Access Coordinator to join our team. This role plays a vital part in promoting positive health outcomes and eliminating health disparities within the community.

The successful candidate will have a minimum of one to three years of experience in outreach and healthcare, or case management. An associate's degree is preferred but not mandatory. A valid Connecticut driver's license and active certification in good standing are required.

The salary for this position is approximately $45,000-$60,000 per year, depending on experience. Benefits include 401K with a matching employer contribution, medical, dental, vision, HSA Spending Accounts, Life Insurance, Short-Term Disability (STD)/Long-Term Disability (LTD), and an Employee Assistance Program. Eligible employees also receive ten paid vacation days, six sick paid days annually, and two personal days paid annually.

Key Responsibilities:
* Assist individuals in selecting the most suitable health insurance plan according to their needs and budget
* Provide education and support to prospective enrollees regarding the health selection recovery process
* Focus outreach efforts on underserved communities, particularly African-American and Hispanic populations
* Ensure clients understand the appropriate role of navigators and provide information in a culturally and linguistically appropriate manner

Requirements:
* Associate's degree or equivalent experience in outreach and healthcare or case management
* One to three years of experience in related field
* Valid Connecticut driver's license and active certification in good standing
* Excellent communication and organizational skills, with ability to work easily with people of diverse backgrounds
* Proficient MS Office and computer skills for reporting requirements, data entry, and paperwork
