Position Overview

About the Role of LPN/RN Hospice Admission Specialist:

  • This is a full-time role with potential for weekend shifts as patient census increases.
  • We are dedicated to providing exceptional care and services to individuals at the end of their life journey. A genuine compassion for hospice care is essential for success in this position.

Key Responsibilities of the LPN/RN Hospice Admission Specialist:

  • Conduct comprehensive assessments of patients and their families, addressing physical, spiritual, and psychosocial needs across various settings including hospitals and home environments.
  • Effectively communicate the hospice philosophy and available services to patients and families, facilitating timely admissions for those who qualify.
  • Collaborate with the Interdisciplinary Team to create and implement an initial patient-centered care plan.
  • Ensure clear communication of patient information to the Hospice Physician for terminal illness certification.
  • Assist with patient evaluations for facility-based contracted beds, manage symptoms, and coordinate transfers as necessary.
  • Work with attending physicians and hospital staff to develop a cohesive care plan that aligns with hospice principles for both new and existing patients in hospital settings.
  • Support other departments and activities as needed to guarantee comprehensive care for all patients.
  • Engage in Continuous Quality Improvement initiatives to enhance the quality of services provided by Ohio's Hospice.
  • Mentor new staff members to foster professional relationships and team cohesion.

Qualifications for LPN/RN Candidates:

  • Must hold a valid LPN or RN license in the State of Ohio with no disciplinary actions.
  • Preferred experience includes a minimum of two years in nursing.
  • Current CPR certification is required.
  • Proficient computer skills for accurate documentation of services and care.
  • Ability to drive in various conditions, including daytime, nighttime, and inclement weather.
  • Possession of a valid Driver's License with a safe driving record.
  • Must meet state minimum automobile insurance requirements.
