Job Summary

We are seeking a highly skilled Care Coordinator to join our team at BHP of Central Ohio. As a Care Coordinator, you will play a critical role in providing care coordination services to adult clients with substance abuse and mental health issues.

Key Responsibilities
  • Implement monitoring systems to ensure clients receive necessary treatment and support.
  • Determine clients' needs and develop personalized care plans to address their unique challenges.
  • Collaborate with other service providers to ensure seamless care coordination and delivery of services.
  • Provide education and support to clients and their families to promote healthy behaviors and outcomes.
Qualifications
  • Strong written and oral communication skills, with the ability to effectively collaborate with other service providers.
  • CMA certification is required, with an Associate's Degree preferred.
  • CPR/BLS certification is also required.
  • Basic computer, phone, and typing skills are necessary for this role.
