Hospice Care Professional

At Capital Caring Health, we are seeking an experienced Hospice Care Professional to join our team. This is a challenging and rewarding role that requires a strong background in nursing and a passion for delivering high-quality care to patients in their homes.
About the RoleThis position involves providing and arranging for hospice care to patients and their families, working collaboratively with the interdisciplinary team to assess patient needs and develop individualized care plans. The successful candidate will have excellent communication skills, be able to work independently, and have a strong commitment to delivering compassionate care.
The Admissions Nurse will be responsible for supervising LPNs, CNAs, Unit Secretaries, and Volunteers, and may be asked to rotate to other regions or departments as needed. A driver's license and access to a vehicle during working hours are required.
Key Responsibilities- Provide and arrange for hospice care to patients and their families
- Assess patient needs and develop individualized care plans
- Supervise LPNs, CNAs, Unit Secretaries, and Volunteers
- Rotate to other regions or departments as needed
To be considered for this role, you will need:
- A degree in Nursing from an accredited nursing program
- Licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice
- Healthcare Provider BLS (CPR) certification
- A driver's license in good standing and access to a vehicle during working hours
We offer a competitive salary range of $85,000 - $115,000 per year, depending on experience, as well as a comprehensive benefits package that includes health insurance, retirement savings plan, and paid time off.
