Job Summary

We are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a Primary Care Physician Assistant, you will be responsible for providing high-quality, patient-centered care to our patients in their homes.

Responsibilities
  • Provide primary care to high-acuity patients in their homes, Monday through Friday
  • Work collaboratively with our support team to ensure seamless care delivery
  • Develop and implement personalized care plans to meet the unique needs of each patient
  • Conduct thorough assessments, diagnose, and treat patients in their homes
  • Order and interpret diagnostic tests, prescribe medications, and provide education to patients and their families
  • Stay up-to-date with the latest medical research, guidelines, and best practices in primary care
Requirements
  • Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located
  • Federal DEA license/eligibility
  • Geriatric training/experience preferred
  • Active CPR/BLS certification
