LHC Group is seeking a highly skilled Home Health Occupational Therapist Assistant to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional care to our patients in their homes.
Responsibilities- Under the supervision of an Occupational Therapist, you will be responsible for providing occupational therapy to patients in their homes to restore them to their fullest physical ability.
- You will perform your duties in accordance with policies and procedures and the established plan of care.
- You will instruct and aid patients in evidence-based treatment within the scope of the occupational therapy assistant and according to state laws governing occupational therapy assistants.
- You will observe, record, and report to the supervising OT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition.
- You will coordinate care with other members of the healthcare team as appropriate.
- You will instruct patient, family, caregiver, and other members of the healthcare team in the areas of therapy within the scope of the occupational therapy assistant.
- You will visit patients within the entire service area of their respective agency and other nearby LHC agencies as needed.
- You will complete a progress note for each visit and submit progress notes per LHC policy.
- You will adhere to LHC Therapy Standards of Practice in provision of service to all patients.
- You will participate in staff conferences and committees as requested by the agency.
- You will have schedule flexibility to ensure we provide care to the patient as needed.
- Current OTA licensure in the state of residence.
- Current CPR certification.
- Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation.
- AL: Must have two (2) years experience practicing as an Occupational Therapist.
- CT: Must be certified by the American Occupational Therapy Association.
- District of Columbia: Not allowed to practice as an Occupational Therapy Assistant.
- IN: Must be certified by a national therapy association.
- LA: Must have a minimum of two (2) years experience as a licensed Occupational Therapist.
- NY: Must be certified and registered with the state of New York.
- SC: Must be certified and in good standing with the National Board for Certification in Occupational Therapy.
