Job Opportunity

Your Tailor Made Senior Service is seeking a skilled Registered Nurse to provide high-quality care to our patients in the comfort of their own homes.

Responsibilities:

  • Deliver comprehensive nursing care to patients in accordance with their individualized plans of care
  • Conduct thorough assessments and develop, implement, and evaluate plans of care in collaboration with the patient, physician, and care team
  • Ensure the safe and effective delivery of home health care services, adhering to all applicable laws, regulations, and policies
  • Collaborate with the interdisciplinary team to develop and implement patient care plans
  • Document all patient interactions, communications, and care activities in accordance with agency policies and procedures
  • Prepare and implement care plans for Home Health Aides, as needed
  • Manage nursing treatments and activities to meet the unique needs of each patient
  • Monitor patient progress and evaluate the effectiveness of care plans, making adjustments as necessary
  • Other duties as assigned

Requirements:

  • Valid RN license in Texas
  • OASIS/Medicare Certified Visits experience
  • Electronic Charting experience with Finale Health
  • Developed computer skills
  • Minimum of 1 year of recent home health nursing experience
  • Driver's License and Vehicle with Current Auto Insurance
  • American Heart Association BLS certification
  • Valid U.S. Driver's License
  • Strong problem-solving and communication skills
  • Positive and professional attitude
