Job Summary

We are seeking a skilled Physical, Occupational and Speech Therapist to join our team at Your Tailor Made Senior Services. As a Home Health Therapist, you will play a vital role in delivering high-quality patient care in a home health environment.

Key Responsibilities
  • Design and implement individualized plans of care for patients, incorporating participation from significant others when appropriate.
  • Provide direct patient care, evaluate functional needs, and consult with other specialists to ensure optimal patient outcomes.
  • Conduct therapy practice within defined standards of care, directing the assistance of support staff to achieve patient goals.
  • Communicate effectively with other professionals and support staff to achieve positive patient outcomes.
  • Document evaluations, treatment goals, and plans via Your Tailor Made Senior Services EMR system(s)-Kinnser, regularly updating documentation and maintaining prescriptions and signatures.
  • Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops, keeping up to date on professional literature.
  • Provide direction and leadership to staff, acting as a resource to promote physical therapy in the community.
  • Ensure compliance with all applicable laws, regulations, and agency policies and procedures.
  • Participate in interdisciplinary team meetings to review and update patient care plans.
  • Communicate with the agency's intake department to facilitate new patient assessments.
  • Participate in ongoing training and education to maintain current knowledge and skills in physical therapy practice.
  • Collaborate with the agency's management team to maintain quality patient care and achieve agency goals.
  • Provide guidance and support to other members of the therapy staff, such as therapy assistants.
  • Maintain accurate and up-to-date patient records and documentation via Your Tailor Made Senior Services EMR System(s)-Kinnser.
  • Maintain electronic patient files to ensure HIPAA standards.
  • Communicate in a professional manner with all parties, including office staff and field workers.
Requirements

To be successful in this role, you will need to have a strong background in physical therapy, occupational therapy, or speech therapy, with experience in home health care. You will also need to be able to work effectively in a team environment, communicate clearly with patients and staff, and maintain accurate records.
