About the RoleThis is a PRN position, which means you will have the flexibility to choose your own schedule and work at your own pace. As a Home Health Registered Nurse PRN, you will be responsible for providing high-quality, patient-centered care to our patients in their homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with our interdisciplinary team to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare providers to ensure optimal care outcomes.
- Current RN license in the state of practice.
- One year of experience as a Home Health Registered Nurse or in a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills and proficiency in electronic medical records.
