Job Summary

We are seeking a skilled pediatric home health nurse to join our team. As a pediatric home health nurse, you will provide high-quality clinical care to medically fragile children in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities

  • Provide skilled nursing care to medically fragile children in their homes.
  • Develop and implement individualized care plans in collaboration with the patient's healthcare team.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Administer medications and treatments as ordered by the patient's physician.
  • Monitor patients' progress and adjust care plans as needed.
  • Communicate effectively with patients, families, and healthcare team members.

Requirements

  • RN/LVN with a current valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
