About the Role

Horizon Home Care & Hospice is seeking a detail-oriented and customer service-focused Intake Specialist to join our growing team. As an Intake Specialist, you will play a critical role in coordinating the screening and intake of Home Care, Supportive Home Care, and Hospice referrals.

Key Responsibilities
  • Coordinate the screening and intake of referrals, ensuring accurate and timely processing.
  • Communicate with referral sources, patients, and staff to obtain necessary information and resolve any issues.
  • Enter patient information into electronic health records accurately and efficiently.
  • Manage incoming referrals, including fax, phone, and email, and contribute to obtaining necessary information to accept referrals.
  • Provide exceptional customer service, responding to phone calls and relaying messages to appropriate team members and departments.
Requirements
  • High School Diploma or equivalent.
  • 2-3 years of office experience, preferably in a healthcare setting.
  • Familiarity with MS Office and medical terminology.
  • Superb communication, organization, prioritization, and customer relation skills.
