Nurse Care Coordinator for Elderly and Disabled Services

Job SummaryWe are seeking a skilled LPN Assessment Nurse to join our team. The successful candidate will be responsible for conducting initial assessments and reassessments for members/clients, ensuring that they have access to the necessary services to remain in their community. The role involves communicating regularly with administrators, documenting assessment entries into Harmony, and participating in team meetings to discuss client progress.
Key Responsibilities- Conduct initial and annual assessments in client/member homes, determining ability to complete IADLs and ADLs.
- Complete modified assessments as needed for transfers, hospital and nursing home discharges, and increased or added services.
- Ensure members have services to remain in their community and fulfill fiduciary obligations based on guidelines.
- Communicate monthly with administrator to receive re-assessment schedule for SOURCE and CCSP annual review.
- Review medications for appropriateness, noting new medications, dosages, and discontinued medications.
- Document assessment entries into Harmony system within established guidelines.
- Participate in team meetings with Medical Director to present approved new admits, reassessments, hospitalizations, Home Health admits and discharges, and repeat variances.
- Attend and participate in mandatory in-services.
To be successful in this role, you will need:
- Nursing degree from an accredited college or university, or a graduate of an accredited/approved license practical nurse (LPN) program.
- Minimum of two years practical nursing experience, with experience working with elders and disabled populations required.
- Current and unrestricted Licensed Practical Nurse (LPN) licensure in state of practice.
This role requires regular attendance, flexibility to meet agency needs, and ability to work effectively in a fast-paced environment. You will be required to maintain confidentiality and adhere to company policies and procedures.
SalaryThe estimated salary range for this position is $60,000 - $80,000 per year, depending on location and experience.
